A Pre-Start-Up Safety Review (PSSR) is a structured, systematic inspection and verification process conducted before any new or modified plant, process unit or facility is commissioned and brought into operation.
What is a PSSR?
The PSSR is the final safety checkpoint before a process facility goes live. It verifies that the plant has been built as designed, that all safety systems are installed and functional, that operating procedures are in place, and that personnel are trained.
In regulatory terms, the PSSR is mandated under OSHA 1910.119 (Process Safety Management of Highly Hazardous Chemicals) and is recognised under CCPS RBPS as a critical management system element.
Why does it matter?
Skipping or rushing a PSSR is one of the most common contributing factors to serious incidents during plant start-up. The Texas City refinery explosion in 2005 — which killed 15 workers and injured 180 — was linked, in part, to inadequate pre-start-up verification of process systems and procedures.
The cost of a delayed start-up, regulatory fine or incident far exceeds the cost of a thorough PSSR conducted in the weeks before commissioning.
What does a PSSR cover?
A well-conducted PSSR addresses six core areas:
1. Construction and equipment verification — confirming the plant was built in accordance with design specifications, P&IDs and safety standards.
2. Pressure safety system verification — pressure relief valves, rupture discs and emergency vent systems are installed, tested and sized correctly.
3. Safety-critical element (SCE) verification — the SCES has been identified, maintained and is ready for operation.
4. Operating procedures — pre-start-up, normal operation, emergency shutdown and abnormal situation procedures are complete, reviewed and accessible.
5. Training and competency — all operations and maintenance personnel have been trained and assessed.
6. Incident investigation and change management — outstanding punch-list items, change notices and outstanding HAZOP actions have been resolved or formally deferred with documented risk acceptance.
